Diabetes Self-Management Education Classes

Managing diabetes takes more than medication — it takes knowledge, practice, and support. That's why Nashville General Hospital offers Diabetes Self-Management Education (DSME) classes led by our certified diabetes care team. Classes meet once a month in a four-hour session and cover everything from blood sugar monitoring and healthy eating to understanding your medications and building a daily routine that works for your life. Sessions include hands-on instruction by a registered dietitian and are open to patients with any type of diabetes. Enrollment is referral-based. Ask your primary care or endocrine provider to refer you, or contact our clinic to learn more.

Know Your Numbers: Understanding Blood Sugar

One of the most important parts of managing diabetes is understanding your blood sugar — and knowing what your personal goals should be. Your care team will work with you to set targets that are right for you. 

Key numbers to know include your A1C (a measure of your average blood sugar over the past 2–3 months) and your daily glucose readings. Most adults aim for an A1C below 7%, but your provider will help you set goals based on your unique health needs. 

Regular monitoring helps your team identify patterns, adjust your treatment plan, and catch problems early — before they become serious.

What should I bring to my first appointment?

Please bring a valid photo ID, your insurance card, a list of current medications, and any recent lab results or imaging studies. If you monitor your blood sugar at home, bring your glucose meter, insulin pump, or CGM device. If your insurance requires a referral, please bring that as well. Plan to arrive 15 minutes early to complete any necessary paperwork.

Do I need a referral to be seen at the Endocrinology Clinic?

Some insurance plans require a referral from your primary care provider before seeing an endocrinologist. Please check with your insurance before scheduling. If you're unsure, our clinic staff can help you figure it out — just give us a call at 615-341-4488.

How do I prepare for my visit?

Preparation depends on your reason for the visit. For diabetes appointments, bring your blood glucose logs, glucose meter, or CGM device. For thyroid or hormone concerns, your provider may ask you to complete lab work before your visit. Our team will let you know in advance if there's anything specific to prepare.

Do you offer diabetes education classes?

How often will I need to come in for follow-up visits?

It depends on your condition and treatment plan. Patients managing diabetes are typically seen every 3–6 months. Thyroid and other hormonal conditions may require a different schedule. Your provider will recommend a follow-up plan based on your individual needs.

What if I have an urgent concern or a blood sugar emergency?

If you are experiencing a medical emergency — such as very high or very low blood sugar that is not responding to treatment — call 911 or go to the nearest emergency department immediately. For urgent but non-emergency concerns during business hours, call our clinic at 615-341-4488.

Can I get lab work done as part of my visit?

Lab work may be completed onsite or ordered through an affiliated laboratory. Your provider will let you know where to go and what tests are needed. Results will be available through the HealtheLife Patient Portal.

Is telehealth available for endocrine and diabetes visits?

Telehealth may be available for certain follow-up appointments. Please contact the clinic at 615-341-4488 to find out if your visit qualifies.

How do I request a medication refill?

Refill requests can be submitted through the HealtheLife Patient Portal, by calling our clinic during business hours, or by asking your pharmacy to send a refill request. Please allow 3–5 business days for processing, and request refills before you run out.

What insurance plans do you accept?

How do I schedule or cancel an appointment?

Call us at 615-341-4488 during business hours to schedule, cancel, or reschedule your appointment. We ask for at least 24 hours' notice whenever possible.
